ATTENDANCE PROCEDURES

    Submit an absence using our attendance form below or use one of these other two methods.  

    Please be specific about the reason why your child will not be attending school today. In the "Reason for Absence, Late Arrival, or Early Dismissal" blank space on the form below, indicate whether your child has any of the symptoms that are listed.

      Complete the form below and click the submit button. This information will be sent directly to the school. The email address that is used in the form will receive a copy of the submission.

      This attendance form is to be submitted by the parent/legal guardian only.
      Are you submitting a full day absence, a late arrival, or early dismissal?
      Please check this box if you wish to report consecutive day absences.
      Will the student be returning to school?
      If reporting an ill student, include symptoms, especially if they have fever, nausea, vomiting, sore throat, and/or cough. If your student has a diagnosis from a healthcare provider, please include that information.
